Silver Linings provides each client with a Long-Term Care Referral Agent Disclosure and Advisory Form and obtains affirmative consent before sharing personal information.

  • The disclosure is necessary to provide placement or coordination services
  • Required by law

Consent may be revoked in writing at any time.

4. How We Use and Share Information

We use personal information strictly for care coordination and referral purposes.

Silver Linings relies on this information to make appropriate recommendations and is not responsible for outcomes affected by incomplete or inaccurate information.

6. Communication Authorization

With client consent, Silver Linings may communicate with:

  • Care facilities

  • Service providers

This communication is limited to information necessary to coordinate care and placement services.

7. Information Protection and Security

We implement reasonable administrative, technical, and physical safeguards to protect personal information against:

These records are maintained securely and used only for operational and compliance purposes. Signed disclosure and advisory forms are retained for a minimum of three (3) years in accordance with state guidelines. Sensitive health and financial information is not retained longer than necessary.

While we strive to recommend appropriate options, we do not guarantee the services, care, or outcomes provided by third-party providers.

Clients have the right to:

Requests may be submitted in writing and will be addressed in a timely manner.
