Silver Linings Senior Care Advisors LLC is a senior living referral service that assists individuals and families in identifying and connecting with long-term care options. Our services include:

• •Personalized consultations to assess care needs and preferences
• •Referrals to assisted living, memory care, independent living, nursing homes, and other senior care communities
• •Coordination and support throughout the placement process
• •Ongoing guidance during care transitions

Our services are provided at no direct cost to clients. Silver Linings receives compensation from care communities upon successful placement.

3. No Guarantee of Outcomes

While we make every effort to recommend appropriate care options, Silver Linings does not guarantee:

• •The availability of specific communities or facilities
• •The quality, safety, or suitability of any referred provider
• •A specific placement outcome

Silver Linings does not own, operate, or control any care facility or provider to which referrals are made. All care and services are the sole responsibility of the respective provider.

4. Client Responsibilities

Clients using our services agree to:

• •Provide accurate, complete, and up-to-date information about care needs and preferences
• •Inform Silver Linings of any changes in health, financial status, or care requirements
• •Review and sign required disclosure and consent forms prior to receiving referrals
• •Make their own informed decisions regarding placement and care

5. Disclosure and Consent

Prior to receiving referrals, clients or their authorized representatives will be provided with a Long-Term Care Referral Agent Disclosure and Advisory Form in accordance with Oregon Department of Human Services (ODHS) guidelines. By signing this form, clients consent to:

• •The sharing of relevant personal information with referred care providers
• •Silver Linings receiving compensation from care communities for successful placements

Consent may be revoked in writing at any time.

6. Privacy and Confidentiality

Silver Linings is committed to protecting the privacy of client information. Personal information is collected, used, and shared in accordance with our Privacy Policy, which is incorporated into these Terms of Service by reference.

7. Third-Party Providers

Silver Linings acts solely as a referral intermediary. We are not responsible for the actions, omissions, quality of care, or outcomes provided by any third-party care community or facility. Clients are encouraged to:

• •Visit and evaluate potential care communities independently
• •Ask questions and request additional information from providers
• •Consult legal, financial, or medical professionals as appropriate

8. Limitation of Liability

To the fullest extent permitted by law, Silver Linings Senior Care Advisors LLC shall not be liable for any indirect, incidental, consequential, or punitive damages arising from:

• •Use of our referral services
• •Decisions made based on our recommendations
• •Services, care, or outcomes provided by referred third-party providers

Our total liability in any matter related to our services shall not exceed the amount, if any, paid directly to Silver Linings by the client.

9. Intellectual Property

All content on the Silver Linings website, including text, graphics, logos, and images, is the property of Silver Linings Senior Care Advisors LLC and is protected by applicable intellectual property laws. Unauthorized reproduction or distribution of our content is prohibited.

10. Website Use

When using our website, you agree not to:
